The following form is displayed when the applicant selects Parking Permits.

Data is entered as follows:
Name (First, MI, Last) - Enter the first name and press [Tab]. The cursor will advance to the next field. If applicable, enter the middle initial and press [Tab]. The cursor will advance to the last field. Enter the last name.
Job Title - Enter the applicant's job title (e.g., Teacher, Secretary, Analyst).
Office - Click on the down arrow to the left of the field. Scroll through the displayed list until the desired location is located. Highlight the location by placing the cursor over it, and click the mouse.

The default is for a school. To enter the office for a location that is not a school (e.g., DIIT, Office of Pupil Transportation), click the circle to the left of Offices. |
License Plate - Enter the license plate for the vehicle for which a parking permit is being requested.
Vehicle Registration # - Enter the 13-character vehicle registration from the New York Stat Registration document.
Contact Telephone # - Enter the applicant's telephone number in xxx-nnn-nnnn format:
Email Id - Enter a valid e-mail address if notification of approval/rejection is to be received in that manner.
The following functions can be performed from this form:
Login - A principal can click Login to display a list of all the permits waiting approval. The function allows each request to be approved or disapproved by the principal one at a time. The principal can also delete a request from this list.
Check Request Status - The applicant can click Check Request Status to view the status of the application.
Submit Request - When all of the required information has been entered on the screen, click Submit Request.
Reset - Click Reset to clear the form without saving any of the entered data.
